Fetching MySQL data with Phonegap/JSON/Ajax

Hi there,
We are developping a phonegap app, which we will run on IOS. There we have a search form in order to fetch insurance rates. Here you can see the php form, which works on broswer IE, Chrome etc.

This form we need in the app under Page 4 and the results have to be shown in page 5.
We already have created the form in the app and the MYSQL File with the SQL Query with encoding. So we need somebody who can write a JSON/Ajax script to return the data in the results page.
