80455 Form with File Upload

Free BSD is operating system. Adults only. You must be 18 years old or older to bid on project or visit site. This is a repost. The last programmer was unable to complete project, so it's only halfway complete. As a result, here are my rules and expectations: 1. Make this form work on my server: [login to view URL] 2. Perl/CGI ONLY! (don't bid with php, etc. options) 3. I have uploaded the form script that the last programmer modified. 4. Once I receive the script and html, upload it to my server, test it, and everything works on all browsers (netscape, ie, aol), then I will transfer payment. NOT that it works on *your* server, but that it works on MY server. 5. So I know you read my rules and agree to them, start your bid off with "Hi Allen" and I'll consider your bid.
