Create a very simple program in html/javascript/php

I have to go to a resort for 2 days and I need someone to create this very simple code for $2. Here's my problem. I just need someone to create three drop down lists. The second dropdown list will display the values based of the choice from the first drop down list and the third dropdown list will display the values based of the choice from the second drop down list. And after that's done, get the values of first, second, third dropdown's values and select from the database where the row that they're all present and display the row in a table. I think this would require 20-30 minutes since basically all I'm asking for is a dependent drop down list then after that take all the values from three of them then create a sql statement where three of them are present and print the entire row into a table. Other info such as db name, table name, table columns (around 5-6) or other stuff will be given when you're hired.
