Easy - Create a form that works on our server

This should be very easy for an experienced form coder.

Attached to the bid are? 2 .php? pages with two different forms that need to be coded to work using the attached [url removed, login to view] code.

I've also attached another .html? form from a different website? that already works on our server that you can copy and use. NOTE: this one already works. We need to model the .php pages after it.

This should be pretty easy.

I need it done in the next couple of hours so don't bid unless you are ready to work. I'm ready to hire you now.
