Adding a button to a title bar?

I want to run an exe as a service, which adds a button to a title bar on an application which I do not own. I then want to respond to button click. I am not wanting an application or a large lump of code for this (unless you have code that works perfectly). I would quite prefer it if you had Yahoo! IM and I would like you to talk me through it and act as a type of consultant. I do not want this done in MFC.
