landing page to capture email

hi i need to create a landing page to capture email leads with [login to view URL] and after that send to my affiliates link, fill the another form and press submit. [login to view URL] the idea it to keep two list but if make some sales get my money from commissions. the landing page its the same in the link. flow: user visit my page ---> fill my list and click submit --> the website redirects to [login to view URL] fill the form and automatic press the submit button its important that we send to affiliates link to get the sales record.
