Website Install/Templatize

We are looking for someone to finish configuring our development website and to templatize our new webpage using php. We have an existing site developed in XHTML and CSS, we need the site to be templatized to include a header file with a constant logo and email address/phone number field, as well as a navigational image that varies depending on the visitor's place in the site. We also require a navigational bar that allows navigation to six sublevels of the site. Additionally we require a sidebar that displays our login box and a subnavigational bar whose contents vary depending on the user's selection from the primary navgational bar. Each page should also contain a separate xhtml body element that is easily editable by dreamweaver for in-house editing. Each page should also contain a footer element listing copyright information. In addition to this templatization, the site must be configured on our new development server such that our existing database search pages (already programmed in php, mysql) work (currently working on a live site of slightly different design) correctly. The server is running Gentoo Linux, Apache2, php4 w/ pear:db and mysql.
