Customized Web-based Javascript Organizer -Medical

We are looking for a web-based "organizer" to be used to create medical care plans online. Essentially the medical care plan data structure is very simple -- a list of headers, each with a list of associated "bullet points" of data. (see attached file for an example) The headers need to be customizable, both for label and order, as do the individual data "bullet points." There are other Javascript-based free- or subscription-based online organizers that have a similar "feel" to what we're looking for ([login to view URL], [login to view URL], [login to view URL]), but none of them have the exact functionality we're looking for. See the attached file for a (very crude) mockup of the kind of layout and functionality we're looking for. Some of the highlights would be: * individual user logins * the ability to add-and-click headers and data elements quickly * the ability to reorganize headers and data elements (moving them up and down in a list) just by dragging and dropping them * the ability to "clone" care plans for templating purposes Both the front and back ends should be compatible with our current webhosting solution, DigitalSpace (see [login to view URL] for current Java, MySQL versions and other site applications already hosted/hostable.). If the project is successful, with a good working relationship between us and the bidder, we would consider re-hiring the bidder for phase 2 of the project (primarily "tweaking" and adding small bits of additional functionality.) I have not posted a programming project before at eLance, so I'm probably leaving out something. :) Please post whatever questions that would help clarify the project. ## Deliverables 1) All deliverables will be considered "work made for hire" under U.S. Copyright law. Employer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the employer on the site per the worker's Worker Legal Agreement). 2) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 3) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Employer's environment--Deliverables must be installed by the Worker in ready-to-run condition in the Employer's environment. b) For all others including desktop software or software the employer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this project. ## Platform MySQL, HTML, JavaScript
