Simple PHP Form Data To JPEG Image / PDF on Webpage

I need a simple web page(s) coded in php to do the following process.
(This form creates a custom Santa letter from a stored image template)
- 1st page will contain a form which people fill in and submit.
- The form when submitted would input the text into a new jpeg file which can be downloaded.
- I also need a button/ piece of code which could convert the new jpeg into pdf for download.
A very close working example would be [url removed, login to view] (It doesn't have a PDF saving option which I want)
- This form must be able to include the text on the image at coordinates similar to the layout on the above site.
- The text output must be able to reference external fonts (In this case Google web fonts) to place into the image.
The Project Files I have attached include details of the exact form I need, The image template to be used and a
description of the exact process (including sample santa letter text) that needs to be completed by the php scripts.
The winner of the project will then be send images for the buttons on the pages being created.
