Wordpress Form Creation & Header Customization

I have a wordpress blog, and I need to add a pop-up form and a form within body with submission confirmation.
The theme installed is:

It allows for insertion of forms via shortcode.
1) The first form will be a button that will be placed on the top right of the navigation. When a user clicks on the button, a form will popup and allow the user to complete and send. Similar to "Get A Lawyer Today" button on:

Here is another similar header button with form ("Call me back"):

The button can also be placed within body of templates. Similar to "get a lawyer today" button on jacobyandmeyers site above.
2) The second form will be created via Contact Form 7 and inserted in the body via Shortcode.
Form will look very similar to:

3) I would like to update the header as such (please see mockup attachment):
a) The blue strip at the top
When you login into WP. Appearance > Them Options > Header
You will see a field called "Call Us Text"
- The text in the blue strip will be displayed from this field.
- Please align the text left
- Please use font style shown in mockup
- The blue strip will be part of the fixed/sticky header. So when you scroll, it will stay as part of the fixed header.
b) Update the font and spacer for the header menu as per mockup (i.e larger text, | separator, etc.).
c) Minimize the HEIGHT of the fixed/sticky header.
Necessary Requirements:
- Forms must be mobile compatible
- Form and fields must be editable via Contact Form 7. Plugin is already installed.
- Styling, CSS, etc. must be completed as part of the form (similar to samples above).
- A "Thank You" prompt in the popup once form is submitted. This already exists as part of contact form 7.
- Data sent must be securely to email
- A copy of the data should be saved in case there is a problem with email submission
- The body form should be available as a widget.
- The popup window will close once data is submitted.
