Mobile compatible Dynamic online form to database (via browser)

We're looking for a mobile device compatible form to database solution which will help our client collect valuable information in the field. I'm looking for a rough estimate at the moment, and will provide some basic information to give you an idea of the scale and capabilities required. Once my client approves the price range, I'll provide more detailed information. I'm sorry, but if you don't offer at least a sample pricing guide, I won't contact you. I'm looking for a fixed price please. There will be various teams traveling around a large area taking inventory of specific roadside objects. We need to have an online form form built that can be accessed via the mobile device being used (tablet or telephone), which will be completed by each of the teams each time they visit one of the objects. Many of the fields will be pre-populated with choices to eliminate errors with entry. So drop downs, text fields, date pickers etc will be required. The data we have already includes IDs and GPS coordinates etc. We'll insert all of this information into the records before this gets taken out the field, so drop down options are pre-populated. When the system is taken out onto the field, the user will select the object ID, which will then bring up all relative information we have previously entered. The user will then complete the rest of the information to fill in the blanks. Some of the dropdown options will trigger further options to be available. As an example, if I chose car from the dropdown, a form asking for the specific car specifications would appear. A pop up, or slide out for this will be fine. Once the secondary (pop up) form is complete, this will disapear to allow the user to continue completing the original form. All data must be save to a central sql database. Each record added will create it's own ID. There is no need for emails, but the information must be viewable, be able to be searched an filtered, be updatable (via the original populated form), and allow for reports to be created and generated. CSV and PDF would be ideal. I would suggest a list of all entries with pagination would be great. A link to the full form details on each listed entry. GPS coordinates will be found and added/updated manually. It would be great to be able to access a google map based on these coordinates when viewing the listing after it has been saved (later when the data is filtered/viewed). Images will also form part of the data collected. There will be 6 images on each object, each with a maximum file size of 3mb. because of their large size, we feel these may need to be uploaded in bulk, manually once the teams return back to the office so they don't have to wait for the uploads while out on the road. Perhaps a predefined image URL/path can be saved to the database, and the file names be inserted into the form instead. When the images are uploaded in bulk later to a specific directory (FTP perhaps), the images will then link up and be available for viewing as part of the form view mentioned above. Image thumbnails, downloads and a preview via a lightbox would be useful here. The form will not need captcha, but since this will be online, we'll need logins and user management control. The team responsible for each record creation, and the creation date/time will need to be noted on each record. The first form will have approximately 25 fields. while the four secondary (pop up/slide in) forms will have an average of about 70 fields. A multi page form will also be fine if required. This form will be used in about 30 different regions. Each of these regions could have 600 entries via this form, so the database needs to handle multiple entries being created at the same time, and need to be able to handle the 18000 plus records that will be created.
