PHP, jQuery : Import file using field mapping in UI

Develop web application (or component) to import events from a csv/excel file using visual field mapping (like zoho CRM or Campaign monitor does in importing..). User importing the file can map the columns from the import file to database columns in the UI Attached are the requirement specifications and SQL script to create the 3 tables with some sample data which are used for import. You need to develop it as a separate application with its UI and deploy on my server to test. This will later be integrated in to the actual web application which will be a separate project and not included in this.
