We have a small web project that involves creating a web based form that we can access from anywhere. This form is really for virtual assistant or call center rep to key in a callers information, and submit to our client. We need the form to flow very simply where we have the following:
Rep Login and Authentication (so we can track who submitted what and record in database)
- Should have users stored in database, with username, password, rep name, and rep email
Once rep logs in they can then submit call requests...
- Enter callers first name, last name, email address, zip code (Validate against USPS zip database or zip code service), automatically populate city and state from this information, grab users street address 1
-- if no email a script should automatically make one on a disposable email service...
There should be a dropdown box for the type of inquiry...
Then submit, once this data is submitted it should be stored in the local MySql database....
A second form based on their drop down response will show up. There are 8 different pages that could show up, all with unique information and requirements.
Rep should be taken to the corresponding form based on the type of inquiry, they complete that form, then the system should submit all information to local database, and to the corresponding remote api. We should then grab a success or failure code. If success let user know success, if fail allow for correction of data and resubmission, abandon, or submit to manager.
We have a copy of the api and info. It is attached to this. We need this turned around in 2 days.
