pHp/mySql database with web interface : Urgent requirement!

Hi, I need a database built with a web interface. It will be a very simple database both in look and feel and application. Basically, a user will log on to a site through a web server login (it should support multiple logins) and see multiple entries in a database. The database will have roughly 8 fields, 7 of which are read only, and 1 the user will be able to click that will bring up a box with some pre-entered text. The fields and the pre-entered text will be added through the back end. On the front end web interface, limit showing 20 entries per page, so if there are 100 entries, it has 5 pages. Think of it as a job website like monster that shows a 100 jobs in the database with read only fields like posted date, company name, salary, etc. And there is one for job descrittion that opens a new window that has the pre-entered job description. I have attached two files of screenshots of something very similar that I need. Screenshot 1 (file name 'Database screenshot') is the database I'm talking about. Screenshot 2 (file name 'Open text box') is the box that opens up with some text in it. There is really no need of a search function either; I just need to be able to add as many as 5000 entries broken down into 3 groups. So in a drop down if one group is selected, it brings up 1000 entries, and 2000 entries each for group 2 and 3. I need this built right away so contact me asap. Strong preference to folks in or close to Chicago as that's where I'm located and would prefer to be able to interface with you in person but not a hard requirement.
