To develop;
1. Relational Database with all PHP for website including neat frontend
2. Password restricted with 4 levels of security (admin, and 3 other levels)
Patients visit doctors. Doctors update patient record at each new consultation through ready made forms (tables). There are many different types of tables for different types of patient information ie contact info, type of disease, type of treatment, type of surgery etc. All data is linked to each specific patient through a hospital number. Data can be added, modified, deleted only by user that entered data or by admin.
When logging into system there must be an easy search method for a patient and when patient is selected, all stored data on that patient should be easy to view and change if necessary.
I already have the tables with appropriate fields and I can give developer the Table structure with all the necessary fields in raw MySQL form. Some fields require upload of documents or images as some tables have images or documents attached.
