Website/Programming/Form Creation

I had a developer working on this project but she was unable to complete it. A Joomla CMS website is at its beginning stage, housed on a Windows 2008 Server/IIS. This site is for secure data entry. They will access this site via VPN. I have several forms to create but only need you to create the first one (or fix the current one the developer was working on) and forward me instructions on how you created it so I can create the others. I need simple and easy, so anyone with no knowledge of this can follow along and create another form; or copy the 1st form and make changes to create the other forms. (My preference would be that you start from scratch, so I’ll know how you created it from beginning to end.) This form has 140 questions (see attached example). The answers typically are (1) text box questions that the reviewer will enter; or (2) Yes, No, N/A radio button questions; or (3) questions where they need to provide a date (pop up calendar to choose date). However, if they answer a question incorrectly, (4) another option will appear that shows the correct answer and violation code which they should be allowed to choose. Also, we need (5) another option here for the reviewer to provide their own comment in a comment field box. All of these options are for one question. However, all of the questions on the form will not have violations. Some questions are just "Yes" "No" questions. There are actually 40 questions with violation codes. There must be an option/button to “save” the data while they are entering it (or a periodically automatic “save”) and once the form is complete, we need a way to lock it so no one can make changes; only the person who submitted the form. The data/answers from these forms should be stored in a mySQL database so the officers of the company can pull the data (including the “violation” data) and export the data to CSV format. Currently, there is no data in the mySQL database, but I have an Excel file populated with the data that I’ll need you to import into mySQL, because the form that you are creating needs to pull the initial data from mySQL. I am illiterate when it comes to this type of work, so forgive me if I’ve said something incorrectly. :-) One last question: the previous developer started on the creation of the first form. It is not allowing the pop up option to select the violation code, nor is there an option to save it periodically. What would you do to fix it?
