Create a template for Gravity Forms PDF

I just need to convert a template into PDF for every generated form entry data collected from the user.
Programmer should know Gravity Forms PDF. My template is currently in PDF as an example, but it should be converted to HTML and then hook all the information gathered in the form.
The form changes a little bit since it depends if it is a male or female completing the form. Some fields change their names and dropdown data info.
