I need a 1 page php/bootstrap/mysql web site with login. Must work on desktop down to cell phone so bootstrap is a must. Reistration and Login with CAPTCHA, main page with 1 data entry page with around 10 data fields with a CAPTCHA and Submit/Print button that produces/downloads a PDF to the user. There will be a 6 tables to populate some dropdown boxes and information entered will be stored in a table. It will also require secure web services to download saved data and delete saved data from main table. Fields with personal information will be encrypted. User will be logged out when closing site. Site will not allow browser to save login and password. Must protect against SQL injection. I will provide database creation script with test data. All you should need to add is registration table(s). This should be enough info for an accurate bid. Further details such as look and feel and 2 page pdf form format will be provided later, once contract is awarded.
