Integrate a sign up process to a wordpress website

My model website is a wordpress site, here is the template: [login to view URL] with this template I created this website: [login to view URL] Everything is great but I was unable to implement this sign up process to my wordpress website: [login to view URL] As you can see, it's not integrated to the wordpress template but it works. it’s in French and You can test it by yourself by clicking "parcourir" to choose an image and "upload" to upload I would need to be integrated to the template design without altering the wordpress updates and the current functions of it. The sign up form is linked to this complete custom made database located here: [login to view URL] The final result should look good on a cell phone, I know it’s a simple task I am asking since everything is already done. You simply need to integrate the form to the template.
