$30 - iPad Web Application in PHP - 30 Minutes

This project should only take an experienced programmer about 30 minutes to complete. We are looking for a very basic, user-friendly web page check-in site that is written in PHP and connects to a MSSQL database server. The user interface web page will run on an iPad inside the Kiosk Pro app. Basic functionality is the user clicks a big Tap Here to Begin button on the first page to start the process, then they iPad keyboard pops up on the screen using Kiosk Pro javascript features (see documentation) and the user will start typing their name into a textbox which will autocomplete their name using AJAX pulling from the MSSQL database of appointments for that day. Once the user selects their name, the web page will have the user verify their phone number and email address or if no email or phone is in the appointment database, the page will have them enter their phone number and email address. Once complete, their check in will be complete and the iPad will redirect to a new first page screen of Tap Here to Begin for the next user. There will need to be a very simple back-end web page to enter new appointments into the MSSQL database for the day. A basic First Name, Last Name, Phone Number, and Email Address are the 4 fields needed. Then an index number and today's date in a date field so the check-in system will only auto-complete from today's appointments. There should be a field that is populated with the current time once the user checks-in on the web page so we know if the user has checked-in and what time the user came in.
