Create a simple single page data entry for that also lists records, saving data to a mysql table.
Single php/html page:
List all from `position` table where status = 'open' order by id
form to add a record (no edit page needed)
button to close a record in the list. (no confirmation needed)
Submitting a new record submits form fields to DB table columns - status defaults to "open"
Closing a record changes status to "closed"
