PHP web page

REQUEST: Produce a single PHP coded web page that generates forms. Text submitted to the forms will not be saved to server. The attached file (form [login to view URL]) has the code that produces forms. These functions were shortcodes developed with wordpress, but for this project wordpress should not be used. A mockup can be viewed at [login to view URL] MOCKUP: The mockup items are as follows: 1. Open a plain text file. 2. Save a plain text file. 3. A large area of plain unformatted text. This is the “soapnote markup” described at [login to view URL] 4. Make an html form. 5. This box displays a form and the output of the form. This displays like the posts at www.soapnote.org. The form should work like the forms at www.soapnote.org. However, there is one feature that does not need to work on this page - the prepopulated text feature (this is explained at [login to view URL]) 6. Form items described in the attached “FORM ITEMS” file 7. Submit button sends form responses to text output. No data needs to be sent to server or saved. 8. Text output that displays after the Submit button is clicked. TESTING: The two attached text files (coumadin adjustment [login to view URL] and abdominal [login to view URL]) can be used for testing the page.
