Video from WebCam

I need a HTML with Javascript. It should create Video with Audio from webcam using JQuery Plugins/any other technique (Can use any opensource dependency).

1. Similar to [login to view URL]

2. Should store the video in .mp4 format in local machine or Directly into a server location.

3. Capture using Adobe Flash plugin is recommended.

4. If possible can provide provision to choose available Camra & Mic.

5. No need of player and all.

My budget is very less. But will give good feedback if completes.

Technology in ASP.NET, JQuery also fine.
