Hi Sir/Madam, I have been working on Office 365 for 7 years. I have experience with SharePoint, Project, PowerApps, Teams, Flow, PowerBI, Planner and Office Plugin. I developed four mobile/tablet apps like inspection, handover protocol, trade management apps. I developed those forms as canvas and model driven apps. I can make the custom connector, form to pdf convert, putting the form into teams tab, etc. I used the Sharepoint, Local Sql server, azure sql server as a database for PowerApps app. Please let me know if you have any questions.

So you want dynamic form which will always get the questions from data source. The form will show question which and it will always get changed when questions are getting changed source...
