I would create a group named "Premium" for example and in admincp change its privileges: - Be able to use larger avatars than normal users; - change the bb code of its group (any style you want, i can make it); - there is an addon that can be found free that will ask the member to "renew" their premium status nor if a user wants to become premium, that allows them to pay directly through paypal nor credit card; About the adsense, in 'FORUMHOME', you can add code and hide it to the groups you want, i used to do this to display messages for some groups (in my old old forum that i used to manage). I dont think you can change the number of replies per thread based on groups, it is set by the user. But i can look into it and try to do it.
