Fire Alarm verification report form (stage 1)

This project is for the first tier of a larger project. This first tier is to create web page or series of web pages to collect data from a buildings fire alarm inspection. This data is currently collected on a paper form (PDF attached to this project) and gets filed in a filing cabinet. The objective of this project is to be able to collect the data electronically and store it in a MySQL database where reports can be generated. For this first tier of the project only the form/report needs to be created. The database and back end scripting will be done later. I am looking for some artistic creativity for the webpage/forms. A plain basic HTML table will not suffice. Also it does not have to look like the attached PDF. The PDF is only there to indicate the elements of data to be captured. I am looking for your creativity to capture this data in the most efficient way possible using a web browser.

In later stages of the project apps for Android and IOS will be created to capture data when there is no network access available. Keep that in mind that this form will be used heavily by mobile devices and should be fully compatible with them.
