ASP / JS / mySQL small easy project

I need to have some work done on my website. The page that needs the work is [login to view URL] as it is a plan can be selected from a number of differant pages and the string is [login to view URL] or something similar. I have most of the work done I just dont have the time to do the rest. I need the plans page completed using a mySQL query to pull the prices for the selected plan I need JS used for the calculations. On Submit I need this information sent to my email. I need the next page to be used for the user to enter contact details. the button on this page will also be the paymanet button which will be a paypal button created using the variables selected on the plans page. The info entered here should be emailed to me and then the user should be taken to the paypal payment page. This should take no more than a few hours to do. Please give me an estimate as soon as you can. thanks.
