Add / delete a field to product registration page

Here is the current URL to our test page: [login to view URL] Looking to be able to add or delete field from the 'Product Information' section on the above form. Would like to be able to add or delete an entire row (Part Number, Serial Number, Install Date) if a client has more than one of our products. Would also need to keep the functionality of the current calendar javascript used for the 'Install Date' field. Also would like the date field to be copied from the first product entry to any new field so that a user would not have to keep choosing a date. However, the user should be able to change the date if needed using the same calendar javascript currently used on the page. Looking for bids on this project, we will supply a copy of the current page and all coding. Sorry, but direct access to our servers will not be allowed.
