Debt Assistance Application

My company is looking for a web developer to deliver a public facing web set and SQL database to store the data captured via that web set. The basis design is attached in the pdf document. The website will need to comply with all accessibility requirements for public users (triple A). The website is to permit applicants to apply for assistance with their debt problems and the design needs to reflect this. There will be two additional pages to the pdf document but will be information text only and no additional fields.
