CSS for Form/Calculator

We recently had a Javascript calculator designed here on Freelancer and now need to style it. It's a very simple calculator that displays 4 text input fields, 2 read only text fields and 1 submit/calculate button. The map results show below the form based on submitted data. We do not have specific requirements for how it should look/feel but we want to have a nice appeal and clean style. Lighter colors, nothing dark.
