Convert PDF to Word Form

I have a 5 page PDF document that I need converted to a MS word form (must be able to fill in as a form and save). The form is 3 pages of the document, the other two are text pages. This must be completed ASAP - I will award the job very quickly. Please indicate in your proposal how long this will take you. I have more than 15 years experience in data entry. I think it is enough two weeks for me to do this work.
