Adjustment to existing DB / Create directory

Existing website, small modification. PHP/MySQL. Copy existing directory(password protected, maintain functionality), change sign up page to add authentification field. Add input field (authentication, member limits to admin section, that sign up field will authenticate against. Add numbering to admin table.) Website is [login to view URL] i.e. 1. Doctor purchases 100 patient limit per month. 2. Admin adds doctor to approved list with 100 patient limit, in admin section. 3. Patient sign up page has field that authenticates against approved doctor & member limit. 4. Admin logs in, and sees doctor & how many patients he has signed up. Admin may also change patient limit. Admin functionality for sorting and deleting already exists. Functionality should be continued with added mods. Added note: Fix left shift of header on the other sign up pages. ## Deliverables 1) All deliverables will be considered "work made for hire" under U.S. Copyright law. Employer will receive exclusive and complete copyrights to all work purchased. (No 3rd party components unless all copyright ramifications are explained AND AGREED TO by the employer on the site per the worker's Worker Legal Agreement). ## Platform PHP, MySQL
