Create form template that can be merged with data files

I have forms in word processing. I have many blank fields in the forms. I want those blank fields to be merged with a data base file. For example, in the forms I have to insert the persons name eight different places. I have to insert their date for birth in four different places. I want to be able to press one button to merge the forms with the data. I need this as a template as I will be doing the merging myself. I just want the templates for the forms and database with the merge function created. I will send over the forms in any wordprocessing program you choose and data sheets.
