Build a interactive flowchart (with design & icons)

We need to adjust a form on our website. We want to replace it using an animated, and good looking, flowchart. The flowchart is made, but it doesn't have any design on it. We want to convert it to a interactive flowchart, using the visual identity of our website. The freelancer should: 1 - Design the concept of the flowchart. It should also include small icons 2 - Implement it using HTML5, and javascript.
