Create a simple form that prepopulates data from button click

I would like a form to be built with 4 fields and form validation - Package:(3 options on dropdown) Name:(text field) Phone:(phone number field) email:(email address field) However, what I want is that on another page, there will be 3 buttons for package: [Option 1] [Option 2] [Option 3] When someone clicks on option 3 button, they get redirected to the form and option 3 is already preselected This will go on wordpress website so please keep that in mind when developing.
