Custom Wordpress App to be build

I have the attached client questionnaire that we get clients to complete when they join with us. We benchmark all of our clients against other clients when they sign up for our consulting package. Currently this questionnaire is in a Excel document (with 7 tabs like the screen shot of the one attached) and we collate all of the clients answers manually. I want a way that clients can digitally complete the assessment, through the 7 stages and then results can be presented to the client graphically. I also want the clients results to be benchmarked against all of our other clients (most likely an average) and also an average for different industries. I want the ability to add more questions and also to be able to manually manage the content and manipulate the results and how these are presented. I am looking for different options on how to best go about this so am interested in quotes to develop this using different technologies. Few parts are left such as the Reporting part and the front end part.
