Facebook design master needed

Please only respond if you have experience making custom fan pages and can show me at least 2 pages you have done in the past. Only bids that PM pages they have done in the past will be responded to. I need someone make my facebook fan page that looks like the same style as this one ===> [login to view URL] I want an original design not a copy of that page. The page I want must match the color scheme on my website. I need an image also designed similar too the one they use for their profile photo. The image will have my company info. I need you to upload it and format the whole fan page, I want to do nothing. Last I want a custom tab designed that will have people request a ebook to be emailed to them. so when they go to the tab there will be a page that asked them to enter their Name and email address. You need to design that page too. When they enter they their info I will be sent and email informing me they have requested the ebook. I need that tab no be able to be viewable unless they become a fan Happy bidding =)
