Build a questionnaire on a website -- 2

I am looking for someone to design a questionnaire for me. I will buy a domain name and host it through whatever platform you think will work best; tumblr etc. I've added pictures of the breakdown of the quiz; yes it's on pen and paper, sorry, I'm on the train and it's a terrible photo. I will obviously give a proper comprehensive flow diagram to you, just wanted to get this posted. The questionnaire is primarily to be used on an iPad but I would like the design to be responsive, and I may design for laptops also if I have time. The questionnaire has seven different pictoral outcomes. The questions determine the users place within a hierarchy; at the point at which the quiz ends for you I want it to link to a page with the relevant image on it and a button to be clicked to print this outcome. The printer may be a polaroid, sticker, or mini printer I am as yet unsure - open to suggestions. I will design the interface (I have most artwork) as soon as I have someone who will work on this and can tell me a bit about what to include (eg should the background be transparent, etc). I'm a final year graphic design student and this is for part of my final major project, which is based on the futility of trying to quantify such subjects (hence the stupid questions). I need this to be completed by 21st May at the latest. Thanks!
