create a dynamic county-level health map of the U.S.

I want to create a dynamic map of health statistics in U.S. counties, building from public data available at [login to view URL] and using the data graphics described at either [login to view URL] or at [login to view URL] Using [login to view URL] would require taking an Excel file and transforming it into a bundled DSPL file Using [login to view URL] would require taking the Excel file and using their applet (at [login to view URL]) There's also free software for creating population density-equalized U.S. county maps, and an example of what I want to create, at [login to view URL]~mejn/cart/ The end-product should allow me to add or subtract data variables, and generate maps and cartograms. I am looking for someone who is already familiar with creating these kinds of graphics and who can provide examples of past work as part of the proposal.
