Business Card style Contact Entry Form

I need a page similar to this: [login to view URL] The page I need will allow visitors to fill in their contact information and their input should be displayed in realtime (using the javascript function "onkeyup" and "onscreen") in a business card template, just like the above mentioned page does. The business card template HAS TO DISPLAY the text using a specific font(maybe using WEFT?): UniversRO Ultra Thin Condensed The font is attached to the project. The page will be part of a joomla site. The data entered should go directly in one of joomla's databases. The project winner will be responsible for business card template design, script development and functionality and implementation and integration into the joomla site.
