Customer Satisfaction Survey

I would like to set up a section of our website where customers, partners and vendors can fill out a customer satisfaction survey regarding the work we have done for them. Questions would be based on a ranking (0 for poor, 5 for excellent) system. Questions would be determined in advance and would be the same for all surveys. I would like the data to be stored in a central database and cross referenced by project so that we can review information by customer, project or partner. In addition, the system would need to have an administration section where we could do the following: 1) Create new survey and fill in some static hidden fields used for reference 2) Create/change survey questions (for entire system, not per survey) 3) Create unique ID that can be sent in an email to our customer/partner/vendor to click and fill out. 3) View aggregate scores by project, customer or partner 4) Export report list to .csv ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). 4) Systems would be confirmed working in a live production environment prior to sign off. ## Platform .asp and Access or MySQL.
