Affiliate program script (repost)

Hello Coders, we are looking for the qualifited coder to develop script on our web site affiliate program for our prize game. Check attached file with detailed decription. Users will klick on affiliate banner, been redirected to our wbe site, Please understand that this is the task - create page web form with the SQL data base where user will fill the form on our web site and sumbmit it in order to winn one of our prizes from the game. Game will have open and closing date. All data should be collected to data central data base. We need ability to sort all received contacts in order to send template predefined commercial mails with attached doc in Pdf format to the data base will be sorted on groups. We need affiliate program to run on the background in order to know where our useres came from to our site. We will promote our game on several web sites and pay for the banner on click. Program must collect this tada so we know how much we have to pay to each affiliate site. We would also like to have statistic about that.
