Convert a PHP content to PDF

Have two kind of pages, one Filling form and the second list of records, I want to add a button to generate a PDF file and to be send automatically by an email to a pre-defined emails in an MySQL database. Database 1- Create 2 tables in MySQL 2- 1 table will have emails and names (Emails) 4- 1 table will store the filled forms (Forms) Web pages 1- Create a form page (PHP) with the fields - Field one Name (select from the 1st table) - Field 1 (anything) - Field 2 (Anything) - Button Send Email (This button should send these contents to the email of the name select and to the default email as PDF with the same layout of the Form) 2- PHP page - Lists all offers in the page - Button to send this list as PDF by email Please use the attached zip file for layout
