I have created a web form in jotforms [login to view URL] however I want to host the form on my own server. The jotform is only an example. I am a non coder website owner so I am looking for the simplest way possible to create and edit multi page forms. Ideally I would like to create a form in jotforms, download the html and then edit the html in a couple of places so that it runs from my server and not theirs. If you think you could do this please get in touch. My budget $200

If the output of the form is to be emailed to you, I can make you a script to recreate the form on your site. You won't need to do anything other than making the form on jotforms and then entering the address into a textbox that the script will show. The rest will happen automatically: it captures the html, makes adjustments, and add php code to handle email sending part. Of course you also need to tell under what name you want it to be saved.
