Create Magento Form

Need a form created using WebForms2 extension that will do the following: 1. User fills out the form. 2. The form then goes to thank you page with a download link. 3. The form generates a PDF with the information that was submitted in the form along with logo and graphics for our company. The download link that appears on the thank you page will direct the user to the PDF. I will supply you with the template for the PDF which includes logo and branding along with placement of the submitted form data.
