Database Required

I need someone to build me a database with a split screen view. The top quarter of the screen will be a form that I will enter the details in, customer info, vehicle details, collection address, delivery address, price charged etc etc etc. The bottom 3 quarters is where the data will be shown. I need the ability to prioritise the order I view the data, ie 1,2 , 3 etc. Each line will represent a job for a particular driver. I have a spreadsheet that I currently use to give you an idea of what I need (see attached). The spreadsheet contains 2 tabs, one for local movements for the current day and future jobs maybe 6 months in advance and the second tab for long distance movements for the year ahead and past. I need the local jobs to be shown in 1 tab and the long distance tabs in another, the same form can be used if that will make things easier.
