Need adobe acrobat expert! Help with completing an online form.

I am in the process of creating an online form with adobe acrobat. I need someone to add signature fields and a submit button that sends the form to my email.
The purpose of this form is for it to be sent to 30+ different clients for them to fill out and submit online (submit button emailing it back to me).
I am looking for someone who can complete this today. If successful, there will be opportunities for more work.
I have attached the form with comments on the final page outlining what i need.
