Hey, I work for a medical association as full time and have worked on similar database, you would not want something using SQL instead of Access ? SQL is more robust and feature rich compared to Access. With Access you will not be able to have a web interface if in the future you wanted a site to access your database from home or on your mobile. Website interface will not only allow for ease of access but also means to showcase what you are doing in your clinic to other physicians. We can work out the scopes of the project and appropriate payment options. Let me know if you would like to consider having the project done using php + sql instead of a standalone MS access based system. Trust me down the road you will want to upgrade to web based system and be platform independent. Regards
