Wordpress site with custom design and functionality

Custom look with wordpress template and must keep all wordpress and template functionality for end user to be able to edit when the site is complete. Slider must have running video in the background, first section that is attached to the slider will have eight different "personalities" with pictures and each must be able to be clicked on separately and a quick form must pop up, the users name, phone and email is to be filled out and the user will receive an email with a quick info sheet attached specific to that "personality." Most important part is they can only fill it out one time. They can not click on the other seven to receive any further information. We will need to be able to capture their information if they try to and/or if they fill it out but do not click the send button. Overall, this functionality is the most important and if you can not complete this then please do not bid on the job. Beyond that important part, this will be a typical website with three different "blogs" that will be able to be found on different pages. We will need to be able to post to each separately. There will be multiple custom sections on the homepage, from quotes with pictures in the background to testimonials to picture sections. Overall expect to build about fifteen pages and be able to customize each as needed. Each picture that is added will need to have the proper alt tags and we expect you to be able to instill the proper SEO into the site. We can provide some pictures but we expect your design skills to be added to the site so please provide the pictures that come from your designing mind. This is not just a straight build, we repeat, we expect from our direction you to be able to take the information we give and provide us with true design elements.
