This project has three parts: 1. Convert an Excel form to an attractive HTML/CSS form. 2. Insert the data into a SQL table using classic ASP script. 3. Load the form with data from a specific row. I have attached an Excel spreadsheet that is has a form to collect data. I would like you to convert this form to an attractive and functional HTML form. When submitted it should insert the data in a MS SQL table via a classic ASP script (it must be ASP script, not PHP, not .net). The table structure is attached and you will need to be able to test on your own server. You will NOT have access to our database for testing. The corresponding SQL column names are shown in red on the Excel spreadsheet. I should also be able to load the form with the data for any specific supplier. For example, if I load [login to view URL] it will populate the data for SupplierID 12 and allow edits. I know that using classic ASP is not ideal, but that is a hard requirement for this project.

Infact, 90% of the projects done so far are in classic ASP, which includes simple dynamic page to complex ecommerce related websites. Are you looking for Delete functionality also? Add and Edit you have already mentioned. Also mention, will it be access only be admin, what I want to know is will there be login functionality before accessing the pages? Please confirm the following flow : a) First page will display like browsing records with the option of edit , delete against each record. b) Button shows on top as "ADD NEW", which will appear as a form (as shown in the excelsheet) after click, and after submit it will come to the first page and record will be added on the top of the records.
